Open A Retirement Account

To open your Pension Gold account, please download the forms below, complete and submit to your nearest FHC branch or to FHC Investments. Do not sign the forms until you present them to an officer of FHC.

Step 1: Assess Your Risk Profile

Step 2: Complete Your Application

  1. Application for Membership
  2. Beneficiary Nomination Form
  3. Supplemental Information Form
  4. Application for Exemption of Contributions
  5. Reference Form – Two references are required (Listing of acceptable referees indicated)
  6. Pension Contribution Deduction Form (applicable if your payments will be made via salary deduction, bank transfers or directly)
  7. FHC Standing Order Authorisation Form (applicable for automatic payments from your FHC Credit Union account)

Step 3: Submit Your Supporting Documents

Along with the documents above, please take the originals of the following with you;

  1. Government Issued Identification (Electoral ID, Driver’s Licence or Passport)
  2. Taxpayer Identification card
  3. Proof of Age (Birth Certificate or Passport)
  4. Proof of Address (dated within the last 3 months)
    • Utility Bill or
    • Statement from a Financial Institution or
    • Letter from a Justice of the Peace or
    • Postmarked Envelope

Note:  Where your passport is used as a source of Identification, you must present a Birth Certificate as proof of age.
